What does a diabetic eye examination involve?
A diabetic eye examination checks vision, eye pressure, the lens and, most importantly, the retina after dilating drops, supported by retinal imaging and an OCT scan of the macula where needed. It finds damage before vision changes, which is when treatment works best. Everyone with diabetes should attend at least once a year.
Why yearly checks matter
Diabetes damages the small vessels of the retina silently. Significant retinopathy and even macular swelling can exist while vision remains normal, and by the time vision drops some damage may be irreversible. Regular examination allows treatment at the stage where it protects sight most effectively.
Diabetes also increases the risk of cataract, glaucoma and temporary focusing changes, all of which are assessed at the same visit.
What happens at the visit
- Vision and refraction, noting that blurring can reflect recent blood sugar swings
- Eye pressure measurement
- Slit lamp examination of the front of the eye and lens
- Dilating drops, then detailed examination of the whole retina
- Retinal photographs for comparison over time
- An OCT scan of the macula if swelling is suspected
What helps your eyes most
Blood sugar, blood pressure and cholesterol control are the strongest influences on whether retinopathy develops and how fast it progresses. Stopping smoking and attending kidney and foot reviews matter too. Work on these with the doctor managing your diabetes, and do not change any medication on your own.
If changes are found
Mild changes usually mean closer monitoring rather than treatment. Macular swelling is typically treated with intravitreal injections, and areas of retina starved of oxygen may be treated with laser. Advanced disease with bleeding or traction may need surgery. Your ophthalmologist will explain what your retina shows and what the next step is.
When to seek urgent assessment
- A sudden drop in vision, a shower of floaters or a shadow across the vision, which needs assessment the same day
- New distortion of straight lines or a blank patch in central vision

- Yes. The purpose of screening is to find changes while vision is still normal, which is exactly when treatment is most effective.
